It's 3:00 PM, your child has just come back from the school playground — and she's complaining of a toothache that's making her miserable. She can't seem to say if there was a particular injury or a blow, but the more she talks about it, the worse it gets. You're the parent... what are you going to do now?
If you've ever been through this type of situation, you know that a calm demeanor and a little TLC can go a long way. But how do you know whether you're facing a dental emergency, or a routine booboo? Here are a few general rules that may help.
First, relax: Without a fever and facial swelling, a child's toothache isn't usually an emergency. But any tooth pain that keeps a child up at night or lasts into the next day should be evaluated by a dentist. Even if it's nothing but a small cavity (the most common cause of toothache) you don't want to let it go untreated. That could allow it to turn from a small discomfort into a major problem — like a painful abscess.
There are some things you can do at home to try and get a handle on what's causing the pain. Encourage the child to show you exactly where the pain is located, and to tell you when and how it started. Then, examine the area closely. Look for obvious brown spots, or even tiny cavities (holes) on biting surfaces or between teeth, which might indicate decay. Also check the gums surrounding the tooth, to see if there are sores or swelling.
You may find evidence of a traumatic injury, like a cut or bruise — or, if only swelling is apparent, it may mean an abscess has formed. If nothing looks amiss, try gently flossing on either side of the hurting tooth. This may dislodge a particle of food that's causing pain and pressure.
If the pain persists, you can try giving an appropriate dose of ibuprofen or acetaminophen, or applying an ice pack on the outside of the jaw — one minute on and one minute off. But even if you can make the immediate pain go away, don't neglect the situation that caused it. Unless you're absolutely sure you know why the toothache occurred, you should bring the child in for an examination. It will put your mind at rest — and maybe prevent a bigger problem down the road.
If you or your family has an active sports lifestyle, you probably already know the importance of food and liquids for energy and hydration. But what you eat and drink (and how often) could unintentionally increase your teeth’s susceptibility to tooth decay. With that in mind, you should plan your nutrition and hydration intake for strenuous exercise to maximize energy and reduce the risk of tooth decay.
On the general health side, carbohydrates are your main source of energy for sports or exercise activity. You should eat a substantial carbohydrate-based meal (such as pasta, cereal or sandwiches) a few hours before a planned event. An hour before, you can snack on something easily digestible (avoiding anything fatty) to prevent hunger and as additional energy fuel.
It’s also important to increase your liquid intake before strenuous activity to avoid dehydration, usually a couple of hours before so that your body has time to eliminate excess fluid. During the activity, you should drink three to six ounces of water or sports drink every ten to twenty minutes to replace fluid lost from perspiration.
While water is your best hydration source, sports drinks can be helpful — they’re designed to replace electrolytes (sodium) lost during strenuous, non-stop activity lasting more than 60 to 90 minutes. They should only be consumed in those situations; your body gains enough from a regular nutritional diet to replace lost nutrients during normal activity.
In relation to your oral health, over-consumption of carbohydrates (like sugar) can increase your risk of tooth decay. The acid in most sports drinks also poses a danger: your teeth’s enamel dissolves (de-mineralizes) in too acidic an environment. For these reasons, you should restrict your intake of these substances — both what you eat and drink and how often you consume them. You should also practice regular oral hygiene by brushing and flossing daily, waiting an hour after eating or drinking to brush giving your saliva time to wash away food particles and neutralize the acid level in your mouth.
Knowing what and when to eat or drink is essential to optimum performance and gain in your physical activities. Along with good oral hygiene, it can also protect your oral health.

A consistently dry mouth is not only uncomfortable and unpleasant but also probably more serious than you think. Dry mouth, medically known as xerostomia (“xero” – dry; “stomia” – mouth) affects millions of people, but few understand why it happens or why it is important.
What Causes Dry Mouth?
It is normal to awaken with a dry mouth because saliva flow decreases at night. But if your mouth is persistently dry throughout the day, it may be a result of habits such as smoking, alcohol or too much coffee drinking or even dehydration. It is also a common side effect of some medications. Xerostomia is not a disease in itself, but it could be a symptom of salivary gland or other systemic (general body) disease.
Why is Saliva Important?
A persistently dry mouth can be a problem. Not only does it feel unpleasant and lead to bad breath, it can also significantly increase your risk for tooth decay. Saliva lubricates your mouth for chewing, eating, digestion and even speaking. Saliva also has important antibacterial activities. Most importantly normal healthy salivary flow neutralizes and buffers acids in the mouth to protect the teeth from the acids produced by bacteria on the teeth that cause decay, and by acids in sodas, sports drinks and juices that can erode tooth enamel.
Not only does saliva neutralize acids but with its high mineral content it can actually reverse de-mineralization — the process by which acids attack enamel and remove calcium from the enamel surface. Healthy saliva actually re-mineralizes the outer layers of tooth enamel, but the process can take 30-60 minutes. That's why it's important not to snack on sugars or drink sodas between meals — one an hour and your mouth is acidic all the time.
Individuals without enough saliva are especially at risk for root decay and fungal infections, and they are also more likely to lose tooth substance through abrasion and erosion.
What Can We Do for a Dry Mouth?

The solution may be as simple as drinking more water and using good daily oral hygiene, or it may necessitate prescription medication to promote more saliva flow.

Dental caries (tooth decay) is similar to the pesky bumblebee that invades your lovely summer barbecue. You can find temporary solace from this intruder by eliminating that very first bee that you see, but if you are situated in an area that is close to the bee's nest, it won't be long before the next bee buzzes along. This is similar to tooth decay. Having one cavity-laden tooth drilled and filled is really just a temporary fix. The underlying conditions that led to tooth decay in the first place need to be addressed in order for your risk of future infection to decrease.
Researcher Dr. John Featherstone created the concept of the Caries Balance in 2002, in which he explained that tooth decay and overall dental health are dependent upon a proper balance of disease-causing and health-promoting factors. Discovering what the fundamental problem really is (and getting as far away from that hornet's nest as possible) can help both determine and curb your risk for future tooth decay.
Here's the issue in a nutshell: Susceptible teeth, in the presence of acid producing bacteria when fed by sugar from your diet, basically, will create all the conditions necessary to cause tooth decay.
To determine your risk for tooth decay, see how many times you answer “Yes” to the following questions:
- Do you brush your teeth twice a day to reduce bacterial plaque sticking to the teeth?
- Do you use fluoride toothpaste to strengthen the teeth against acid attack?
- Do you use a fluoride mouthrinse?
- Do you floss daily?
Every affirmative answer decreases your risk of getting cavities, but even doing all of this may not be enough!
Now, how many times can you answer “Yes” to these questions?:
- Do you smoke? Smoking causes mouth dryness, and creates a host of other health problems.
- Do you snack frequently between meals? One sugary snack and your mouth is acidic for the next hour. One snack per hour and your mouth is acidic all day.
- Do you frequently have acid reflux or heartburn? Reflux creates extreme acidity in the mouth and directly erodes tooth enamel.
- Do you drink soda, sports drinks, or acidic beverages frequently? These beverages are very acidic.
- Is your mouth frequently dry? Do you take any medications that cause mouth dryness? Saliva is nature's own defense against acidity and helps neutralize acid in the mouth.
- Have you had frequent cavities in the past and/or have you had any crowns or fillings in the past three months? The best indicator of future disease is past disease!
Every affirmative answer increases your risk of getting cavities!

One important fact that surprises many people, is the age you should take your children to the dentist for their first dental appointment, age one. The reason that the age one dental visit is so important is that it establishes the foundation of oral healthcare for the rest of their lives. Unfortunately, some parents wrongly assume that because primary teeth “fall out anyway,” they do not need to worry about them. Nothing could be further from the truth!
One problem children may face is Early Childhood Caries (ECC) tooth decay. This is a type of tooth decay that occurs from sucking on a bottle filled with sugary liquids such as formula, juices and fruity drinks for extended periods of time and from a sleep-time bottle.
